The first publication of the data dashboard provides the number of patients attending an NHS Health Check between April 2012 and March 2018, using data recorded by participating GP practices. Data is presented in a dashboard and broken down by age, sex, ethnicity as well as local authority and Clinical Commissioning Group.

Strategic Health Asset Planning and Evaluation (SHAPE)

Strategic Health Asset Planning and Evaluation (SHAPE) is a PHE tool and platform that uses GIS and data visualisation to bring a range of data together to inform delivery as well as evaluate services.  As well as providing a freely accessible interactive tool to support service configuration, known as the SHAPE atlas for CVD prevention, it is built and developed to allow commissioners, strategy makers and decision makers make informed decision in health improvement in their respective localities and geographies.
