Healthier You: NHS Diabetes Prevention Programme
Healthier You: NHS Diabetes Prevention Programme (NHS DPP) is a joint commitment from NHS England, the Office for Health Improvement and Disparities (OHID) and Diabetes UK, to deliver at scale evidence based behavioural interventions for individuals identified as being at high risk of developing Type 2 diabetes.
The aim of the programme is to reduce people’s risk of developing Type 2 diabetes across England. This will consequently improve the health of the nation whilst at the same time reducing a major financial burden on the system; one which goes beyond managing the condition itself, to the potential treatment of a number of associated conditions such as sight loss, kidney failure and stroke.
Referral onto the programme
The Healthier You programme is now available nationally, the NHS DPP will be available to every adult in England identified as being at risk of Type 2 diabetes (now known as Non-Diabetic Hyperglycaemia). We anticipate that people at risk of Type 2 diabetes will primarily be referred into the programme via one of three routes:

- Those who have already been identified as being at high risk of Type 2 diabetes by their GP. Routine primary care appointments can provide openings or opportunities to identify those at high risk Type 2 diabetes and we know that in many cases GPs will already have identified individuals at high risk and will have information on their systems enabling them to refer into the NHS DPP.
- The NHS Health Check programme, commissioned by all local authorities in England, systematically invites adults between the ages of 40 and 74 for risk awareness and assessment every five years and includes a diabetes risk assessment and blood test as part of the check.
- The Diabetes UK “Know your Risk”, this is new to the programme and is a a self-referral route. People can complete the Know Your Risk tool, receive a risk score and depending on their risk, go on to enter their postcode and are put in touch their local provider: https://riskscore.diabetes.org.uk/start
